Responsibilities:
Records Information concerning inquires, admissions and discharges and assists with daily census reportsCoordinates the inquiry process and facilitates the admission process including completing required paperworkAppropriately notifies facility representatives of admission updatesKeep monthly record of census, matching up numbers with Business Office to ensure accuracy at the end of each monthProvide information on referral leads, admissions, and discharges on a weekly basis or as requestedCommunicates effectively with the Social Security Department, Nursing and Business Office regarding bed availabilityEvaluates referrals to determine admission candidacy, including visiting referrals at home or involved with community agency as appropriateMaintain close contact with all referral sources.
Notify hospitals of bed availability regularly.
Associates or Bachelor's degree preferredAdmissions experience in LTC/SNF preferred.
Positive and energetic individual with creativityAbility to work independently or part of a group.
Computer knowledge.
Ability to work weekend(s), when coverage is needed or assisting with marketing eventsDirector of Admissions Schedules &
